PFMLA Consultation FAQ's

How can you help?

I offer sliding scale consultations to assess qualifying medical/personal circumstances that qualify you for leave. The state requires that a qualified health professional verify the condition/circumstances necessitating leave (clinical social workers are qualified health professionals according to the state). Should you choose to consult with me, I will complete the medical certification document and send it to you to submit with your other application materials.

What does the consultation/assessment process entail? 

You will complete brief intake documents online. I will spend 30-90 minutes with you (phone or video conference) discussing the conditions necessitating PFMLA, and how much leave you need. I may also offer brief interventions/suggestions/referrals to support your wellbeing, if requested and consented to. After our meeting, I will complete documentation and my portion of the medical certification form. I will ask any follow up questions as needed. I will then deliver the completed medical certification form via the online portal, which you can the  submit to the state with your leave application. 

How much will the consult/document completion cost me?

The cost for the service is $300. There are sliding scale options available ($0-300) for those who need it. You can pay upfront, or we can set up a payment plan. No insurance is accepted at this time.

If you are Black and/or Indigenous, the services is free!

Be sure to identify that you are interested in PFMLA consultation. If you are ready, please also include your preferred fee based on your access to financial resources now and in the near future.

What should I do before we meet?

  • Login to the Tending Roots portal and complete intake documentation. You should have a link in your email. Contact me if you don't.

How quickly can we do this? Will you help with the rest of the process?

If it’s urgent and my schedule allows, we can complete the meeting and documentation in a few hours. I try to respond to inquiries within 24 hours. ​

I can answer some basic questions about the process.  If you need more than brief assistance with the FMLA application process, I can refer you to a colleague and community member who provides sliding scale peer counseling and resource navigation.
